Plan networks change from time to time; if you don't see yours, ask; we may still be in network or able to help with out-of-network benefits.
- Self-pay
Self-pay is welcome too. Rates are simple and flat: $150 for an initial psychiatric evaluation and $150 for a follow-up appointment. We'll always confirm the cost before care begins.

To protect your privacy, we don't collect insurance details through this website. Verifying your benefits is still free and easy; it happens over the phone, by email, or in the secure patient portal.

Send your name, your insurance company, and a good callback number to intake@freedomhealthbehavioral.com and we'll check your benefits and get back to you within one business day. Please don't include member ID numbers or card photos; we collect those securely once you're a patient.
During scheduling
Request an appointment and our intake team verifies your benefits before your first visit, then confirms any copay or deductible with you; cost never arrives as a surprise.
In the patient portal
Existing patients can share insurance card images and updates securely through TherapyPortal, where they go straight into your chart. Please don't email photos of insurance cards or IDs.
