For your privacy, clinical forms are never collected on this website. Everything happens inside TherapyPortal, the secure client site of our records system.
- Request an appointment (online, by phone, or by email) and we create your chart.
- You receive a TherapyPortal invitation by email within one business day.
- Complete, sign, and submit your intake paperwork in the portal before your first video visit.

Photo ID, insurance cards, prior records, referral letters, and completed paper forms are shared securely inside the portal. Please don't email documents or health information; email isn't a secure channel for them.

Portal submissions are protected inside our records system, with safeguards ordinary web forms and email don't have.
Electronic signatures in the portal are valid, dated, and stored with your chart automatically.
The portal works from the same phone, tablet, or computer you'll use for your video visits.
